Zwangsmaßnahmen in der allgemeinen Psychiatrie – Subjektive Perspektiven von Ergotherapeuten zur Fixierung von Patienten
Abstract
Hintergrund und Zielsetzung: Die Zahl an Zwangseinweisungen in Deutschland und damit im Zusammenhang durchgeführte Zwangsmaßnahmen sind in den letzten Jahren explosiv angestiegen. Die Konfrontation mit einer Fixierung stellt für die Patienten häufig eine traumatisierende und belastende Situation dar und hat somit Auswirkungen auf die Therapie. Ziel der Forschungsarbeit sind die subjektiven Perspektiven von Ergotherapeuten zur Fixierung von Patienten im stationären Setting zu erfassen.
Methode: Insgesamt wurden drei Ergotherapeuten innerhalb zwei verschiedener Einrichtungen mittels halbstrukturierten, leitfadengestützten Interviews in einer face-to-face Einzelsituation zur Thematik von Fixierungen befragt. Die Analyse der Transkripte erfolgte anhand der inhaltlich strukturierten Inhaltsanalyse nach Kuckartz mit Hilfe des Softwareprogrammes MAXQDA 12.
Ergebnisse: Es wurde ein differenziertes Kategoriensystem mit deduktiv gebildeten Hauptkategorien und induktiv gebildeten Subkategorien evaluiert. Neben dem hohen Stellenwert von Zwangsmaßnahmen im Bereich der akuten Psychiatrie konnten die Untersuchungsteilnehmer keinen Einfluss zwischen der Durchführung einer Fixierung und den Auswirkungen auf ihre Rolle als Therapeuten feststellen. Sie sehen jedoch Maßnahmen und Methoden, welche speziell der Ergotherapie zur Verfügung stehen, um ein solches Verhalten in Einzelsituationen zu verhindern.
Fazit: Fixierungen nehmen im psychischen Setting und somit auch im Arbeitsfeld der Ergotherapie einen immer größeren Stellenwert ein. Aufgrund von Studienlimitationen sind weitere Forschungen mit größeren Stichproben sinnvoll.
Compulsive Treatments in General Psychiatry - Subjective Perceptions of Ergotherapists on the Fixation of Patients
Background and Objective: Over the past few decades the number of compulsory hospitalizations in Germany increased considerably, and consequently also the number of compulsory treatments increased accordingly. The confrontation of the patient with a fixation is often a traumatic situation and therefore also has an effect onto the therapy. The objectives of this research are the subjective perceptions of the Occupational Therapists relating to the fixation of patients in a stationary setting.
Method: Three Occupational Therapists, who work in two separate Facilities, were questioned individually in a face-to-face interview to give their impressions and experiences regarding the theme of fixation. The analysis of the interview transcripts was done based on a structured content-analysis according to Kuckartz, and using the software program MAXQDA 12.
Results: A differentiated category-system, composed of deductively developed main categories and inductively developed sub-categories was evaluated. Aside from the high significance of compulsory treatments in the area of psychiatry, the study participants could not identify a correlation between fixation and its impact on their role as therapists. However, they recognize that procedures and methods specifically available of the Occupational Therapy could avoid behaviour requiring fixation in individual situations.
Conclusion: Fixations are becoming more and more standard methods in the psychological setting and consequently also in the working environment of Occupational Therapy. Because of the limitations associated with this study further research using a larger sampling is recommended.
